Read the "Hausordnung" or "Badeordnung" on the website of the spa, or call and ask about nudity. Different spas, different rules.
Common rule is you must have a towel to sit on, and she can wrap it around her. Showers will be nude, but can be gender separated. After being there around other women being nude, most women stop caring.

Our first time, in Cologne was amazing, if you like other men seeing your wife naked they will do up close. All the Spas, both in hotels and seperate wellness centres, we have been to have been a good mix of old and young of both sexes.
