If you are going to do anal play, I highly suggest you give yourself an enema. If she is already leery about anal play, she will not be happy if she runs her finger up your ass and hits a shit log. As far as positions go you will have to find a position you can be in for a little while. I have a friend that is a doctor and has a family practice.
