A few years after your post, a retrospective article about the game tracked down the original photographer, who still had a few outtakes in his archives.

Ah, yes. Roberta Williams, co-owner of Sierra Online and creator of the "King Quest" series of games. She and her husband Ken (he might be the dude in the pic but I'm not sure) where famous for producing a lot of the graphic adventure games of the 80s. If it had "Quest" in the title, it was probably a Sierra game.

They were later acquired by Activision.

According to the article I linked above, the waiter was literally an actual waiter at a restaurant down the road from the Williamses' house (where the shoot took place).
