Default A moment from some media
Apparently there is a film called "Skullduggery" that was released in 1970 that has a scene where a character named Douglas tells a character named Sybil the phrase "your fly is open". It might be the earliest known instance of this happening.

Are there any other examples or instances that happened in the past or that could be found through media such as movies and shows? Is that moment as common as it was back when this thread started?
