My wife has had this happen a few times over the last few months. Last night we went to a local place that has old style video games and a club upstairs. It's a 21+ location. She was wearing one of my favorite tops. It's designed for women who are a B/C cup so it doesn't function the same with DDDs. The material crosses under her left breast but because of it's size, it gradually opens up to show underboob for awhile then the material rides up to her nipple. She knows what might happen but doesn't really feel when it opens and definitely can't see it. She can feel when her nipple is free though. She had a number of exposures while we were playing games and while walking around last night. I usually hang a few steps behind her while we are walking to watch guy's reactions. Some try to take in the sight without being obvious while others are obviously looking. She readjusts once in awhile.
Her nipple was exposed as we walked into the club then over to the bar upstairs. The male bartenders didn't say anything. She readjusted before we walked down a few stairs to the dance floor. Her nipple was on the edge of making an appearance after awhile. I could see a few guys watching and probably cheering it on inside. Then a girl near her leaned over to let her know her boob was about to fall out. It was also on the edge of being exposed when she went to the bathroom. A woman in there walked up and adjusted it. That surprised her. She wore the same type of shirt (different color) when we went to play pool a few weeks ago. The waitress let her know she was falling out twice. So it seems a lot of woman will "watch out for each other" while men will just watch. I'll have to get a picture of her in that top someday. Well, sure. As a male, if I told a woman that I could see more than intended, I would be slapped for looking in the first place and how dare I say anything. If a woman mentioned it to another woman, then she would get a thank you.
That being said (I shared this story many, many years ago), I once had a co-worker have a middle button unbutton itself, so her lacy bra was being shown to anyone who would stand off to the side a bit, and some dark nipple could be made out. I noticed it right away but I was too scared to say anything because she had an aggressive personality, and I didn't want her to yell at me for looking in the first place. A few hours later, she came out of the bathroom and told me that her top was unbuttoned, and how could I have not noticed. I told her that I did notice, but I was too scared to say anything. Her response was, "I wish you would have said something. I flashed probably 100 people this morning. And I don't care if you look there. You can look there all you want. So keep looking at my boobs and say something next time!" FWIW, many years later, we became really good friends, and I was hanging out with her and her husband at their house. She was wearing a tank top with enormous arm holes without a bra. I, again, was too scared to say what I could see, but her husband said, "Hey, Gnome can see your boobs there, hun," and she said, "You both know that I don't care!" and I got many nice, long peeks at her super perky boobs all afternoon.

I was at a college football game, I took a break to run take a leak, when I came out of the men's a young lady came out of the women's, she was walking right in front of me. He skirt was caught on something and I could see one entire cheek of her panty clad butt, a very nice view. I said "excuse me miss, your skirt is pulled up in the back" in the most grandfatherly way. She reach back, pulled it down and gave me this nasty glare, I felt like I should have waited until a woman saw her ass.

I was talking to a female friend about this earlier. She said she’s told other women when something was wrong with their clothes. As the discussion went on, I realized I would mention something to her and most other female friends. I say most because I’d take in the view if it was someone I was interested in. Or I’d comment in a way that could be taken as a compliment. I’d probably do the same for other woman depending on the situation. I’ve seen woman who were obviously showing on purpose.
