He's been injected with saline solution. It's somewhat like using a vacuum pump to swell the size of the penis, clitoris or entire vagina. They inject a big syringe full of a saline solution into their scrotal sac, penal sheath or both. Sometimes the penal sheath is inflated so large the glans will be completely hidden by the swollen balloon of skin. I have no idea how they remove it all, other than sucking most of it out with the syringe and then letting their body absorb the rest.
