The Tower of pain, or Devil's Bra.

Strapped round the back of the chest like a bra. As the screw is tightened it pulls the clover clamps away from the chest, both stretching the nipples and tightening the pinch on the clover clamps. Usually adjusted to the point of the maximum pain the subbie can tolerate without tapping out.

It works on tits of any size.

In the first two pics the girl's also wearing a tit crusher, although it doesn't look as if it's been tightened far. The third and fourth pics are of a different woman.
