What makes it harder to identify them is that whilst some Photos are completely AI generated and therefore fake, genuine photos that have been upscaled or enhanced by AI software can demonstrate some of the characteristics of AI photos. Theses can include mis-shapen fingers, lack of grain or texture.
It is also possible that the original photo is real but has been run through one of the many "nudify" apps so it's a genuine photo but a fake nude.
