Tit and nipple play.
The 'tower of pain' has screw adjustments to stretch the nipples away from the chest and hold them there. It's finely adjustable to give just the right amount of pain.
In one of the pics the nipples are in clover clamps so, as the device stretches the nipples, the clamps tighten too. That hurts.
