Extra nipples are fairly common and I think, genetic.
My ex wife had 3 nipples. 2 were normal and normally placed. The extra was smaller than the others and situated at the underboob of her right breast (where her bottom of a normal bra cup would irritate it if she was doing any kind of strenuous activity. She wore a sports bra most of the time for this reason.
No other women in her family admitted to having extras, but her nephew (older sister's son) must have had 13, 14 or even 15 of them on his chest and back as a kid. When he got older, he had then removed.
