Waties Island above Cherry Grove Beach at the SC/NC boarder - little hard to reach by vehicle (as it should be) but easy by boat or jet ski. Locals have used it for years for topless sunbathing and more. I've been there a few times and used it with several women for full nude and sex on the beach, never been bothered as there's never many people there.
