I don't alway do a level adjustment. Most of my work is dodging and burning to bring out already existing highlights and shadows. This prevents color distortion and that nasty pixelation. I then sample the skin and paint it in at 25% increments over the area I'm working on. Then sample the lip color (as this is closest to nipple color) and paint it over the nipples in increments. Instead of using the standard dodge and burn tools, I use the paint brush, set to overlay at 5%-25%, using either black or white and paint over the area. When the detail starts to come out I darken the nipples a little more than the rest to accentuate them. Levels is useful when details are hard to see. This method is easier on lighter colors. For black I start with a levels adjustment and experiment from there. Hope this helps. Have fun.
