I think it's real. Here's my theory on what is happening in this photo:
Our lovely subject was being silly with her friends and gave the "one boob flash" by pulling her top open to the left. Then, when the camera was pointed at her, she didn't want them to catch a photo of her boob, so she just let go of the shirt, expecting it to pop right back into place, seeing as how it is such a snug fitting top (as previously noted). BUT even SHE didn't realize what stiff resistance her uber-perky little nipple would provide, catching the fabric and momentarily preventing her boob from being covered up. In the moments after the photo, there would have been much embarrased laughing as all gathered realized that she thought she'd been clever, only to be thwarted (and exposed) by her own determined nipple.
