I will answer in reverse order:
Model release - Yes, for anything I intended or actually sold.
Going rate - I'm out of the business now but back then there were going rates for various types of photographs based on subject, rights, access to negatives etc. I don't remember what the rate was 30 years ago.
Not happy to see me -
We had a pleasant separation. She was starting to look for more permanence than I was prepared to provide. She told me that this was not unique to me and it took her two years and many guys before she met one who was looking for a longer term relationship.
I later learned that I wasn't the first photographer from her past. I had heard of an incident with Jane which occurred a year or so after we broke up but didn't have any details. What I since heard was that she made a bad decision and was doing something she shouldn't have been doing in a public place and the activity went south. Unfortunately, a paparazzi was in the area and photographed the activity. Paparazzi's today are a mix of good and bad but back in the 80's they were some of the most unethical/immoral people in the world. As a result, this guy waited until she was married and blackmailed her to perform for him. I have no direct knowledge of this but I believed the source. As a result she thought I was going to do the same thing. I wasn't but I was agreeable to her bargain she bartered.
I haven't spoken with Jane in many years so I don't know her current view of me. I think it would be mixed. In looking back there are a couple of things I regret but I can't take them back now.
Photographs-
Yes, while I don't have many since most were sold/bartered away I still have some. A few have leaked and I have actually seen a couple on this site. The leak was caused by a roommate hacking my computer 10 years ago
Any other questions?
