If you make your Flickr photos be available to users you have designated as friends and/or family only, only friends and/or family can see them. Kind of like Facebook.
One can also make photos on Flickr 'private', which means only you, the poster, can see them; it creates some sense of privacy, so that your photos aren't all out there for everyone to see. I do it all the time - I even have photos of ex-gf's on Flickr marked as private that no one else can see them, but I can see anytime - the wife is none the wiser.
