That can happen a lot, the thumbnail preview does not get updated with the actual image is manipulated. This is not the same as un-editing the actual photo. Most times the thumbnail is a much smaller representation of the actual image. You'll notice that in the majority of those cases, the "uncensored" image is either much smaller, or it's much grainier (because someone is trying to resize the thumbnail image to normal size, and there just isn't enough data in the image to make it clearer.
