Google image search gets you to a 140+ set of pics on the fap site, under the name Mijan. Looked through them, and found no indication of nipple-access enhancement. There aren't very many that show the underboob view, but one has a "hint" of a scar on the right boob, but nothing visible on the left one. Other pictures show her exposing these magnificent breasts in various locations, and a few have her hands touching/denting her breasts in a way that makes them look natural. My guess? I think they might be real, but if they are bought-and-paid-for, every woman who gets that surgery should go to this woman's doctor.
