Twice at Disney - Once in the Magic Kingdom to the left of the Castle near the bushes, and once on the monorail.
In a Newport RI Mansion during our tour. We got caught by 4 different people but just kept going.

Capital Grill Boston - While my wife was standing at the bar (It was very crowded due to an event in town) I slipped it up her skirt and gave her a few pumps ..... although I didn't got to finish until later
