i was actually having a conversation with my wife about this over breakfast.
you can see from my posts that she has very large breasts and she has suggested that a breast reduction may be something she will need in the future, but she did say that since she got a bra that fitted properly she doesn't get anywhere near as many back problems as she used to.
so in conclusion, we think that poorly fitted bras cause most of the pain
