This is actually an intentional tactic. My sister used to play waterpolo in high school and told me that they used to pin their costumes to help stop it happening. The idea being that the opponent is not going to keep playing with a boob hanging out and will stop to fix herself giving you a chance to score.

Then it would be more fair if they all played naked
