In my experience, most women think it painful to caress a man's sack or they just never think to do it--concentrating on the shaft instead. My advice is to try to 'train' her. Tell her that you want her to suck your balls, have them washed and shaved and maybe she will oblige. It will be fun to work on it....start by having her massage you!
